Registering a Stillbirth

It is the responsibility of the mother or father/parent of the child to attend the Registration Office. Please take the card provided and your marriage/civil partnership certificate (if available) with you together with the certificate of stillbirth (form6) issued by the doctor or midwife.
Where the parents are not married to each other or in a registered civil partnership, normally they should attend together, if both names are to appear on the stillbirth entry. Alternatively, a Statutory Declaration may be made by either parent to enable the stillbirth to be registered on the information of one parent alone. (These forms are available from any Registration Office and must be completed prior to registration).
You may, if you wish, give a name to the baby at the time of registration. You can, if you wish, obtain one copy of the certificate at the time of the registration. This is free of charge.
Stillbirths that occur in Scotland may be registered at any registration office in Scotland.
