Your Health - Glasgow Learning Disability Partnership
In Glasgow, people's health has been improving over the last fifty years. This is mainly because people are better off, better educated and live in a cleaner environment. Health and social care services have also improved.
Glasgow Learning Disability Partnership is working to help people with learning disabilities to lead healthy lives, and to get good quality health support and advice when they need it.
Primary Care Liaison Team
The Primary Care Liaison Team supports local Area Learning Disability Teams to offer health checks to all adults in Glasgow with learning disabilities. The health check can happen in the person's own home or another venue of their choice, for example a resource centre. The health check covers all aspects of the person's health. After their health check, each person receives a 'hand held record' - this is a book with all the information about his or her health at the time of the check. For more information about health checks you can phone the Primary Care Liaison Team on 0141 211 0830.
