To retrieve details of past examinations records, you can either:

• phone/write SQA to request a form to be posted to your home address
• e-mail customer@sqa.org.uk to request a form
• go to the SQA Web Site to download a form

The form may be posted, faxed or e-mailed.

Please Note: If the form is being e-mailed to SQA, the candidate’s signature must be scanned onto the form before sending.
