How to apply, step by step
To apply for the Glasgow Young Scot Card, you can download and print out the Glasgow Young Scot application form (PDF File 277kb, 8 pages). You will require Adobe Acrobat Reader to access this form. This software can be freely downloaded from the Adobe Website.
You can also ask for a pre-printed application form from your local library, leisure centre or school office.
Alternatively, collect the form from our office. We're open Monday to Thursday, 9am to 5pm and Friday, 9am to 4pm.
Please note the office will be closed on Public Holidays (5, 26 May; 19 Jul; 29 Sep; 25, 26 Dec 08).
Complete and sign the form (if you're not yet 12, you'll need your parent's/guardian's signature). The card acts as a voluntary proof of age ID, so the info on your form must be verified. Your application form must be accompanied by a passport sized photograph. The back of the photograph should be signed by the person stamping the form, and written "I confirm that this is a true likeness of (Applicant's Name)." Carefully follow the instructions on page 3 of the form.

Telling us about a change in circumstances

Don't forget to quote your card number to help us trace your details.
Replacement cards
Look after your card, but if it gets lost or stolen, don't panic. We can issue replacement cards. Complete the standard application form, specifying "Replacement Card".
