Service Review Procedure and tools that are used to plan review activity and document the findings and actions resulting from the process.

This Procedure assists Social Work Service staff to participate in the service review process.  Services are subject to review according to the contract type or service level agreement, but reviews will usually occur at intervals of three years.  Dependant on contract terms, some contracts may also be subject to annual review.

 
Service Review Plan Templates

These will be completed prior to the review and will include details of those personnel to be involved in the process.  Information is captured as follows:

  • Service environment – performance objectives, best practice, strategic and service plans and dependencies relating to other services;
  • Approach – scope, aims and objectives, timescales, quality requirements, available skills, funding, resources and service review controls

This checklist is used during a service review to capture evidence, assist in following a three stage approach and in assessing the five criteria adopted for use across all care and support service types in all service areas:

  1. Cost of the service
  2. Demand for the service
  3. Strategic relevance
  4. Provider capacity
  5. Quality and effectiveness

This template is used to document the findings, conclusions and any recommendations of the review.  It also details the implementation of any agreed actions including timescales for completion.

This document describes the steps to be followed at the findings and actions stage of a service review.  It includes production and circulation of reports, consultation with service providers, outcomes and implementation options in relation to recommendations.
