Joint Community Care Plan

 

Community care is about maximising the quality of lives of vulnerable people by supporting them to live in their own homes and communities.

In Glasgow there are a significant number of citizens who are vulnerable through frailty, illness, a physical or learning disability, mental health problems or an addiction. There are also an estimated 60,000 carers in the city who provide support to relatives and friends, often at cost to their own personal wellbeing.

Our Joint Community Care Plan provides a vision of how community care will develop in Glasgow. It is produced in partnership between the Council’s Social Work Services, NHS Greater Glasgow and Clyde, and Housing Agencies.

 

The current Joint Community Care Plan/Extended Local Partnership Agreement 2004/2007 sets out the vision for planning and delivering community care services in Glasgow City.

 

The Annual Reviews below report on progress made within the 2004/2007 plan:

We are currently seeking feedback on the draft version of the 2008-2011 Joint Community Care Plan.  This consultation is open to all interested parties and has been extended to run until the 2 June 2008.
