Home from hospital is the provision of care to all hospital-discharged patients requiring immediate home care. In Glasgow hospitals, nursing staff can make direct referrals to Direct and Care Services when a patient is due for release and needs help at home during the immediate recuperation period. This enables patients to be discharged more quickly. Our rapid response teams then put in place the necessary short term care requirements.

The duration of care provided by this service is approximately two weeks for each individual after which they are transferred to mainstream home care provision.
