Community Care
Working in Community Care will be rewarding and challenging for Social Workers and Care Managers. Community Care Teams are currently working towards integrated services with health to progress the Joint Futures Agenda. Although there are specialisms, we are encouraging developments in best practice and of managing crossovers between the services.
Community Care Services focus on assessment, care planning, care management and reviewing processes. Social Workers/Care Managers have a critical role in the process and there is a range of operational procedures/relevant training available to support staff.
Area-based Learning Disability Teams are committed to partnership working with health. We have co-located teams at area services and our health partners have joined the teams to assist in the assessment and care management role. Similarly our Older People Teams have been developing services with health in terms of the integration of services. We are developing models for delivery of services to maximise the resources available and deliver effective services.
Adult Services have a remit across a broad range of service users including mental health, physical disability, addiction and palliative care. Our Mental Health Officers for adult services are located within Adult Mental Health Resource Centres. We are keen to promote the developments for integrated services in mental health with our health partners.
The development for Occupational Therapy Services in the Joint Futures agenda will result in the improvement of service delivery which can promote Occupational Therapists role in assessment and care management functions too.
We are committed to collaboration and partnership working. The Service Plan 2001-2004 and the Locality Planning process at area service level will ensure the change agenda is progressed.
