As a Personal Licence Holder you need to undergo refresher training every 5 years for the date your licence was issued. You must provide a copy of your training certificate to the Licensing Board no later than three months after each 5 year training period.
If you have recently completed your refresher training you can use this form to send the Board a copy of your training certificate (SCPLH-R).
To use this form you will need an electronic copy of your training certificate. This could be a PDF or scanned copy of the certificate or alternatively a clear photo of the full certificate is acceptable. If you are using a device with a camera, you will be able to take a picture of the certificate and attach it.
